What to Bring to a Connecticut Workers’ Compensation Consultation
A workplace injury can raise questions about treatment, time away from work and correspondence from an insurer. Before a workers’ compensation consultation, gather the information you already have so the attorney can identify what needs further review.
Describe the work and the injury
Prepare a short timeline with your job duties, the date or period involved, how the injury occurred and when you first noticed symptoms. Identify any witnesses and explain whom you notified. If you are uncertain about a date, say so rather than guessing.
Organize medical records
Bring available treatment notes, appointment information, medical bills and written work restrictions. List the providers you have seen and any upcoming appointments. Tell the attorney about earlier injuries or treatment involving the same body area.
Bring employment and claim documents
Gather pay records, employer information, claim numbers, letters from the insurer and notices of hearings. Include any forms or agreements you have received, even if you do not understand them. Make a list of missing documents to discuss at the consultation.
Explain the current problem
Identify whether your concern involves medical care, missed wages, returning to work, a disputed claim or an agreement you have been asked to consider. Bring the actual correspondence connected with that concern.
Ask how the matter will be handled
Discuss what records need to be obtained, any upcoming dates, communication and fees. The attorney’s assessment will depend on the facts and documents in your matter.
